When I was experiencing level 9 pains in the ambulance, I was offered Fentnyl, I said fuck no. Btw, my appendix was swollen to over 10cm long and 1.6cm width, that's over 2x the girth.
The only other option they offered was ketamine, I didn't want it, but like any good pusher they were persistent and I finally acquiesced.
It was a good 20 minutes before I felt a thing and this stuff was dripping into my veins the whole time. Then the pain began to dissappear but in a very odd way, the discomfort was still there. Like, wow I am uncomfortable level 9 but zero pain. So it wasn't exactly fun.
Then my eyes shut and I couldn't open them, literally no connection to them and my entire vision was replaced with this.

Yep, that's all, and it was fixed.
I told them I wasn't impressed and they laughed.
Then my body began to feel all over numb, the discomfort ended for all of about 3 minutes and the moment that IV ended, the pain began ramping back up. In 20 minutes I was back at 9-10 pain. Literally the worst pain you can imagine.
They gave me some other pain killer, that did nothing, then morphine, that was like setting my body on fire, it did bring the pain down to 8-9 but not until after setting my whole body on fire. It was awful. I was almost screaming in pain.
Also, after every shot I threw up, even when they gave me the anti-nausia, I threw up. There was constant bad nausia, but at least the vomit helped the awful cottonmouth.
Needless to say they yanked the morphine out and tried something else. By this time I was having trouble breathing. Still in 8-9 pain and slowly increasing they gave me something specific to appendix pain and it went to 4. I had to consiously breath. If exgf wasn't there I would probably have had a bad day. They didn't have to give me anything else till surgery and I woke up with less than a 2 pain.
